Forms for scanning can be created from your existing intake instruments, surveys, or questionnaires.

Forms created for the TCU ADC assessment system can be modified upon request.

There are two steps to the process of customization: creating or modifying the actual form, and creating/modifying the form definition file used in scanning.

Two levels of modification are offered:

1. Modification to the wording of the questions, labels, or titles. This type of customization is billed at a rate of $90/hour, (with a 2 hour minimum/form). For example, a request many involve changing the wording of all questions on the MOTForm that use the words "drug abuse" and "substance abuse" to read "alcohol abuse".
2. Modifications involving the insertion of additional questions, changing the number of digits of any of the input fields, etc. This type of customization is billed at a rate of $90/hour (with 4 hour minimum/form). For extensive changes or the creation of new forms, we will provide an estimate. Modifications must continue to conform to the OMR one-page legal-size format.

The same minimum order of 500/box per title applies to all forms customized by MicroWorksForms, and the same quantity discounts are offered.

To request a quote on customization of a form, please submit a document describing/listing the changes you require. A non-refundable deposit will required before work commences.

Even careful rewording should be tested to ensure research validity & reliability for customized assessment instruments.
